Chosen Grant

The Chosen Grant was created to help individuals and families financially with their infertility journey. Our ability to hand out grants depends on donations that we receive throughout the year.

Circle Surrogacy 2024 - Circle Surrogacy

Those in need of financial help with taking the next steps in growing their family through surrogacy are welcome to apply for the Chosen Surrogacy Grant. All applicants must be willing and able to receive treatment through our partner, Circle Surrogacy.

With our partnership, each grant covers the following:

Surrogacy Program Agency Fee (~$38,500)

AND

Journey Protection Guarantee (~$9,000)

All additional expenses for treatment, donor eggs/sperm, medications, elective services/testing such as PGD (genetic testing) as well as the fees associated with the surrogate must be covered by the patient. Refer to the “Cost Sheet Document” below provided by  Circle Surrogacy for additional details.

Chosen Grant funds will NOT be paid to the applicant or his or her spouse or partner; rather funds shall be paid directly to the agency, Circle Surrogacy.

Chosen Grants are limited by the amount of funds received via donations and are awarded as frequently as possible. All applicants will receive an email to confirm submission of his or her application, and certain selected grant recipient(s) will be contacted by Chosen. Announcement of such selected recipient(s) will be highlighted on social media within two months following the grant submission deadline. 

Applicant understands and agrees that submission of his or her application does not guarantee that he or she will be selected for a Chosen Grant.

Eligibility

Applicants must meet all the following requirements in order to be eligible for a Chosen Grant:

– Be a resident of the United States of America;
– Be less than 50 years of age;
– Have a diagnosis of infertility certified by a medical provider, with exception for: Applicants who are single, or part of a same sex relationship;
– Have no infertility insurance coverage or have exhausted benefits. If a patient has coverage for monitoring and/or medications, they will still be considered. Patient(s) would be responsible for any deductibles or copays;
– Must confirm financial stability, basic health insurance, adequate living arrangements, and means for child support;
– Be under the care or willing to be under the care of Circle Surrogacy;
– Be able and willing to start treatment within 3 months following receipt of award; and
– Be willing to receive a background check.

*To match with a CARRIER, INTENDED PARENTS must either have PGT-A euploid embryos or at least a 40% chance of achieving a pregnancy with a collection of non-PGT-A embryos, as determined by the IVF Physician.  Additionally, each individual transfer with untested embryos must have at least a 25% chance of success as determined by the IVF Physician.  If INTENDED PARENTS are joining the program with the intent to use their previously created embryos and will not create more embryos with an EGG DONOR, PRINCIPAL shall request verification of success probability from the IVF Physician. As an example, if INTENDED PARENTS only have one embryo, it must have at least a 40% chance of success, but if they have 2 or more, then each transfer must have at least a 25% chance of success and together they must have at least a 40% chance after transferring all embryos. INTENDED PARENTS with low level mosaic embryos that meet the embryo quality requirements as stated herein may be considered on a case-by-case basis.  INTENDED PARENTS must provide PRINCIPAL with the IVF physician’s required waiver for their CARRIER to sign acknowledging the risks of a transfer of a mosaic embryo prior to matching.  The IVF Physician must also counsel both INTENDED PARENTS and CARRIER regarding the risks of a transfer of a mosaic embryo.

*Considered applicants may be asked for additional documentation and invited to have a consultation with Circle Surrogacy prior to selection to review medical records and treatment plan in greater detail. 

*Chosen Infertility Group follows all HIPAA standards, discarding all private information and materials immediately following the selection of an applicant.

* Chosen Infertility Group will be notified by Circle Surrogacy of the outcome of the cycle.
